Listing the equipment type is especially helpful because authorization for one forklift category does not automatically qualify an operator to use every type of powered industrial truck.

- Review every field for spelling and date accuracy.
- Have the authorized trainer or evaluator sign the completed document.

Do not pre-sign blank forms or issue a card before the operator has successfully completed the required evaluation. If the operator changes jobs, equipment, or work conditions, review whether additional training is needed.
Printing Tips
For a wallet card, print on heavy cardstock and use the printer’s actual-size setting rather than “fit to page.” For a full-page certificate, choose high-quality or best print mode and check the margins before printing multiple copies. A paper trimmer gives cards cleaner edges than scissors. Lamination can protect a frequently handled card, but leave the original record unlaminated so it can be filed and updated.
Before distributing the finished document, compare it with the employer’s training records and workplace requirements. A neat printable supports organization, but the training, evaluation, and recordkeeping behind it are what make the certification meaningful.
