A printable forklift certification wallet card template gives employers a compact way to document that an operator has completed required forklift training and evaluation. The card can be carried in a wallet, badge holder, or safety binder for quick reference on the warehouse floor.
This type of card is useful for warehouse managers, manufacturing supervisors, construction companies, distribution centers, and safety trainers. It can also help employees show their supervisor which equipment they are authorized to operate. A wallet card is a practical summary, but it does not replace the employer’s complete training records or the hands-on evaluation required for forklift operation.
What to Include on the Card
Use clear labels and leave enough room for handwriting or typed information. A useful template should include:
- Operator name: Use the employee’s full name.
- Training date: Record when formal and practical instruction was completed.
- Evaluation date: Add the date the operator demonstrated safe performance in the workplace.
- Trainer or evaluator: Identify the qualified person who conducted the training or evaluation.
- Equipment information: Add the forklift class, type, or model when helpful.
- Employer details: Include the company name, location, phone number, or employee ID if desired.
OSHA does not require a specific wallet-card design. The important information is the employer’s certification record showing that the operator was trained and evaluated. The card should accurately reflect the records kept by the company.
How to Use the Template
- Enter the operator’s information only after the required training and workplace evaluation are complete.
- Confirm that the equipment listed matches the type of forklift the employee is expected to use.
- Have the responsible trainer, evaluator, or supervisor review the details.
- Print one card for the operator and keep the supporting training documents in the employee or safety file.
- Update or replace the card whenever refresher training, a new evaluation, or a change in equipment authorization occurs.
The card should never be issued as a substitute for training. Employees should not operate a forklift based only on a printed form, online lesson, or general work experience.
Printing Tips for a Durable Wallet Card
Print the template at its original size rather than using “fit to page” unless the layout specifically calls for scaling. Standard wallet-card dimensions are close to 3.5 by 2 inches, so check the print preview before producing multiple copies. Print on heavy cardstock instead of ordinary printer paper. Use a dark, readable font, leave sufficient writing space, and test one card before printing a full sheet.
After cutting the cards, laminate them to protect against moisture, dust, grease, and frequent handling. A small paper cutter helps create straight edges. Keep a blank backup copy and compare every issued card with the company’s official training records.
